Knowing your options can save you time and money. Please collect the following information and documents for all household members:
- Legal name
- Birth date
- Identification number (Social Security Number or immigration information)
- Relationship to you (spouse, mother, child)
- Income before taxes Tax filing status (this is what you report on IRS tax forms)
- Proof of income if your income has changed since you last filed taxes
- Current health insurance information (coverage dates, group number and policy number)
- Proof of immigration status if you are a legal permanent resident
- Proof of Tribal status if you are a Registered American Indian/Alaska Native

I need to make a change to my policy (I'm with INW)
In order to make a change to your policy prior to its renewal date (January 1), you must have incurred a "Qualifying Event." A Qualifying Event triggers a Special Enrollment Period (SEP)
What is considered a Qualifying Event?
If you have incurred a Qualifying Event and need to make changes to your policy, please contact Roxanne.
What is considered a Qualifying Event?
- Losing existing health coverage, including job-based, individual, and student plans
- Losing eligibility for Medicare, Medicaid, or CHIP
- Turning 26 and losing coverage through a parent’s plan
- Changes in household
- Getting married or divorced
- Having a baby or adopting a child
- Death in the family
- Changes in residence
- Moving to a different ZIP code or county
- A student moving to or from the place they attend school
- A seasonal worker moving to or from the place they both live and work
- Moving to or from a shelter or other transitional housing

How do I create a Washington Health Plan finder account?
In order to apply for insurance through the Washington Health Plan Finder you will need to first set up an account:

Set up a User Name and Password for your account along with email and security questions. At the bottom of page choose “CREATE ACCOUNT”.. Continue to add information, once you have set up the account, let me know by calling 253-661-0054 or emailing me at roxanne@insurenw.com.
I will also need the last four digits of your social security number to attach the account to me as your Agent/Broker which will allow me to help you now and in the future.
